Leaving your little dog with a friend, a neighbor or a family member is easier when the person knows what your dog is used to. This printable Care Passport lets you write down your dog's daily routine, favorite walks, the words your dog knows and the little quirks only you know, so your helper does not have to guess.

It is a personal handover guide, not an official document. It is not a travel passport, a vaccination record or a certificate, and it does not replace instructions from your veterinarian.

How It Works

4. Go through it together with your helper before you leave.
